Registration forms (print only)

In order to be registered with the Penryn or one of our branch surgeries at Stithians or Mawnan Smith, please follow one of the two steps below :

1) Come along to the surgery where one of our Receptionists will ask you to fill in a GMS1 Registration form and a Patient Medical Questionnaire (or GMS1 Registration form and a Student Medical Questionnaire - if you are a student at Penryn campus).  Hand these back to the Receptionist at the Surgery.  The Receptionist will then arrange for you to be fully registered with one of our Doctors.

2) Print off the relevant Medical Questionnaire form and GMS1 below, fill them in, bring them along to one of the surgeries and hand these in to the Receptionist.  The Receptionist will then arrange for you to be fully registered with one of our Doctors.

FAQs :

1) Home address :

On the GMS1 Registration form, your home address is where you are residing at the present time.  If you are a student, this is your university accommodation address, not the address from where you have come or your parent's / guardian's address.

2) Can I register online ?

At the present time, this facility is not available.  The forms have to printed off and brought in or posted to the surgery.  Alternatively, come along to one of the surgeries and we can supply the relevant forms for you to complete.

3) Why do I have to fill in a Medical Questionnaire ?

We ask you to fill in a Medical Questionnaire because it can take a considerable time for us to receive your medical records and if treatment is required in the meantime, your doctor or nurse will have some background medical information about you.

We have a "Patient medical questionnaire" and a "Student medical questionnaire" as there is slightly different information required for each cohort of patients. This is purely an administrative requirement and in no way compromises the medical care received by any of our patients.
